Notice of Data Security Incident

Springdale, Arkansas – April 22, 2024 - The privacy and security of the personal information we maintain is of the utmost importance to Arisa Health Incorporated (“Arisa Health”).

On or about March 18, 2024, we experienced a network security incident that caused disruption to some of our systems. Upon learning of this issue, we immediately commenced a prompt and thorough investigation. As part of the investigation, Arisa Health has been working very closely with external cybersecurity professionals experienced in handling these types of incidents.

Our investigation is in the earliest stages and is still ongoing. As such, we are not one hundred percent certain whose personal information may have been impacted. However, in general, the potentially impacted information could include protected health information (PHI). We have no evidence that any personal information has been or will be misused for identity theft or medical/financial fraud as a direct result of this incident. When the investigation concludes, we will notify individuals whose information is present in the impacted files. The potential information impacted could include full name, address, date of birth, phone number, Social Security number, medical record number, health plan beneficiary number, patient identification number, financial account number, medical diagnosis information, health insurance information, health insurance policy number, and/or subscriber identification number. Once our investigation concludes, we will update this list.

Notified individuals will be provided with best practices to protect their information, and individuals whose Social Security numbers are contained in the impacted files will be offered complimentary credit monitoring. Notified individuals should also review the preventive measures outlined in the “Other Important Information” section.

We are committed to maintaining the privacy of personal information in our possession and have taken many precautions to safeguard it. We continually evaluate and modify our practices to enhance the security and privacy of the personal information we maintain.

We have also established a call center to address questions from impacted individuals. Representatives are available Monday through Friday between the hours of 8:00 a.m.-5:00 p.m. CST at (866) 528-1113.
