This summary provides a brief overview of the employee benefits provided to full-time employees working a minimum of 30 hours per week at Arisa Health. We have worked hard to make our benefits package comprehensive, and we value these benefits that are employer-sponsored to be around 20-22% of your total compensation.

New employees are eligible for benefits after a 60-day waiting period. You can include your eligible dependents: your spouse and children up to age 26.

BENEFIT

COVERAGE OPTIONS

MEDICAL

• ABCBS Standard HDHP Plan – Plan pays 80% for most covered services after deductible of
$4,000 per individual/$8,000 family limit (offset by HSA funds)*

• ABCBS Enhanced PPO Plan – Plan pays 80% for most covered services after deductible of
$3,000 per individual/$6,000 family limit. This plan also offers PCP copay $30 and SPEC copay $50.*

• ABCBS Premium PPO Plan – Plan pays 80% for most covered services after deductible of
$2,000 per individual/$4,000 family limit. This plan also offers PCP copay $20 and SPEC copay $40.*

DENTAL

• Delta Dental Standard Plan – Provides services for Preventive, Basic and Major dental care up to $750 per year. Excludes orthodontia.

• Delta Dental Premium Plan – Provides services for Preventive, Basic and Major dental care up to $750 per year. Includes orthodontia up to $2,000 lifetime max.

VISION

• VSP Vision Plan – Includes an annual eye exam ($10 copay then 100%) and lenses every calendar year.

HEALTH SAVINGS ACCOUNT (HSA)

Employees enrolled in the ABCBS Standard HDHP Plan can participate in the WEX Health HSA, based on IRS eligibility rules:
  • Individual Coverage – Contribute up to $4,150 per year.
  • Family Coverage – Contribute up to $8,300 per year.
  • 55+ – You may contribute an additional $1,000 annually.

FLEXIBLE SPENDING ACCOUNTS (FSA)

Enroll in the WEX Health FSA to pay for health and dependent care expenses with tax-free dollars:

•Healthcare FSA – Contribute up to $3,200 per year through pre-tax payroll deductions for eligible medical, dental and vision expenses.
  • Participants in ABCBS Standard HDHP Plan are eligible to contribute to the Limited Purpose Healthcare FSA for dental and vision expenses only.
•Dependent Care FSA – Contribute up to $5,000 per year for dependent care.

BASIC LIFE AND AD&D INSURANCE (COMPANY- PAID)

• Prudential Life & AD&D – 1 X base annual earnings up to $150,000.

DISABILITY INSURANCE


• Prudential Short-Term Disability (STD) –
Replaces 60% of weekly earnings (maximum of $2,500 per week) up to 11 weeks. This benefit is 100% paid by Arisa Health.

• Prudential Voluntary Long-Term Disability (LTD) – Replaces 60% of monthly earnings (maximum of $7,500 per month) up to Social Security Retirement Age.

EMPLOYEE ASSISTANCE PROGRAM (EAP)


Health Advocate 
is also our new vendor for EAP. You have access to virtual counseling, face-to-face sessions, online webinars, and course catalog to aid you on your wellness and mental health journey.

We also still have the Prudential Life EAP through GuidanceResources provides no-cost, confidential counseling and support for a wide range of personal issues, such as stress and emotional health; substance abuse; parenting and child or elder care; financial coaching; legal consultation; and more.

401(k) RETIREMENT SAVINGS PLAN

Take advantage of saving for retirement through the 401K Retirement Plan through Voya Financial. Participants can defer through pre-tax or Roth contributions (2024 maximum of $23,000 plus catch up of $7,500 if you are over 50) and are eligible for enrollment after 1 year of service and age 18 or older. Safe harbor employer contributions will equal 3% of your compensation with 100% immediate vesting (regardless of whether you make a deferral contribution). An additional employer contribution will equal a $1 for $1 match up to the first 3% of your deferrals. You can also roll prior retirement assets into the plan at any time.
