Arisa Health Client Portal Access Agreement



 
Arisa Health offers secure viewing and communication through our Client Portal as a service to clients who wish to view parts of their records or to communicate with our staff.  Through the secure messaging function, clients will be able to send and receive emails for non-urgent needs. The Client Portal offers the following functions:

  • Send and receive secure messages to/from our staff – such as non-urgent medical questions, follow up information, or reminders.
  • View and print “Continuity of Care Document” or your care summary.
  • Request medication refills. View your current medications list entered by your provider or clinical staff during you most recent visit.
  • View selected health information (allergies, medications, current problems, past medical history.
  • View certain lab results (lab results that contain sensitive information will not be available in the portal).
  • View and update your demographic information (i.e. address, phone numbers, etc)
  • Complete registration and health information forms.
  • Read patient education information.
  • View and request appointments.
  • View account information.

Secure messaging can be a valuable communications tool, but has certain risks.  By signing the Agreement to Abide by the Terms of Use, you accept the risks and agree to follow Terms of Use, as described below.

    • The Client Portal is a web page that uses encryption and other security measures designed to keep unauthorized person(s) from reading communications, information, or attachments.  
    • Secure messages and information are designed to be read only by someone who knows the right password or passphrase to log into the Client Portal.  
    • The Client Portal is provided “as is” and no guarantee is made regarding the ability to access the portal at any particular time, location, or internet speed.

Medical Advice and Information Disclaimer:

Arisa Health Client Portal may from time to time include information posted in the form of news, opinions, or general educational materials that should not be construed as specific medical advice or instruction.  The information posted within the Client Portal should not be considered a complete health record, nor should it be relied on to suggest a course of treatment for a particular individual. You should always seek the advice of your provider with any questions you may have regarding a medical condition or result and you should never disregard treatment advice or delay in seeking it because of something you may have read or interpreted on the Client Portal.

The Client Portal is not intended to replace an office visit.  Please do not send messages through the Client Portal that require immediate medical attention.  For any urgent matters, please contact your provider’s office.

  • *The information in the Client Portal is not the official complete health record.  If you need additional health information or your complete record, please contact the Health Information Department at 870-972-4900 for details regarding the release of your health information. Communications May become a Part of Your Health Record

We make every effort to ensure the accuracy and integrity of information or material that we produce or provide through the service.

  • IF YOU HAVE ANY QUESTIONS OR CONCERNS ABOUT THE PERSONAL INFORMATION IN YOUR ELECTRONIC HEALTH RECORD, PLEASE CONTACT YOUR PROVIDER.

Registration for the Client Portal:

To register you must be at least eighteen (18) years of age.  Persons under the age of 18 may be registered by a parent or guardian.  In some instances, a minor may register, such as in the case of a legally emancipated minor, after providing Arisa Health with the documentation or legal proof that is required before they will be permitted to register.  For more information, you can contact the Director of the Health Information Department at 870-972-4046.

 

Prohibited Conduct:

User agrees to use the service for lawful purposes.  User agrees not to

  1. Post offensive or inappropriate material on the portal or the use of rude or threatening language;
  2. Abuse, defame, harass, or stalk any individual or staff of Arisa Health
  3. Reproduce or copy for your personal use any text, photographs, graphics, icon buttons, images, artwork, names, logos, and trademarks or service marks contained in the portal
  4. interfere or attempt to interfere with, damage or attempt to damage, the service or the proper working of the service
  5. use any process, to access, acquire, copy or monitor any portion of the portal, or in any way reproduce the navigational structure to obtain any materials, documents or information through any means not purposely made available through the portal
  6. Misrepresent your identity, provide false information, impersonate another person or entity, misrepresent your affiliation with a person or entity
  7. Attempt to obtain unauthorized access to the portal or use the service to gain unauthorized access to anyone else’s information
  8. Probe, scan or test the vulnerability of the portal, or breach the security or authentication measures of the service
  9. Assist any third party in engaging in any activity prohibited by these Terms of use

 

Privacy and Security:

Arisa Health reserves the right to limit sensitive information, related to behavioral health, genetic testing, substance abuse, or HIV/AIDS information, which it opts out not to include on a portal due to concerns with inappropriate access, lack of patient understanding, or in accordance with state laws and federal laws. Keeping messages secure depends on two additional factors:

  • (1) the secure message must reach the correct email address.
  • 2) The authorized individual must be able to get access to it.  

 

Only you can make sure these two factors are present.  It is your responsibility to make sure that we have your correct email address and inform Arisa Health if it ever changes.  You also need to keep track of who has access to your Client Portal account, so that only you or someone you have authorized can see messages received or other information in your Client Portal.  You should protect your Client Portal login information from anyone whom you do not want to access your portal account and notify us immediately of any unauthorized use of your login information or if you believe that your login is no longer confidential.  You will be solely responsible for maintaining the security of your password and if you lose, share or otherwise make your password available to others, the confidentiality of your protected health information may be compromised. For example, if you post your password on your monitor or save your password on your computer, others may gain access to your health information. The User assumes all risk relating to the user’s viewing of health information on the user’s computer or mobile device and the transmission of health information via a third-party network, such as the user’s internet service provider. You understand that if you print any of the health information it is your responsibility to safeguard the printed records and any protected health information contained on those printed records. You understand that your health information may contain information related to mental health, substance abuse, or HIV/AIDS treatment and Arisa Health will not be responsible for any disclosures the user intentionally or unintentionally makes to third parties.

 

All email address lists will be kept confidential and such lists will not be shared with other parties, unless necessary to carry out Client Portal operations (e.g. perform system upgrades to the portal) or required by law.

 

We will not answer questions or send protected health information by regular email.  Even with these security measures, we cannot guarantee the confidentiality, security or integrity of the Client Portal information.  

 

To the fullest extent allowed by law, you agree to not hold Arisa Health, its physician practices, its providers or any of its staff liable for network infractions beyond our control. Please read our Notice of Privacy Practices for additional information on uses and disclosures.

 

Response Time:

The Client Portal is checked during hours of operation, which are from 8:00am to 4:30pm Monday through Friday. You are encouraged to use the portal at any time; however, messages submitted after hours are held for us until we return the next business day. Response time may be longer if the Client Portal service is interrupted for maintenance, upgrades, or emergency repairs and other related events beyond our control.  Arisa Health will not be responsible for service interruption beyond our control.

 

Client Acknowledgement and Agreement:

When you electronically “Accept” the Client Portal Terms of Use agreement, you acknowledge that you have received and read the Client Portal Terms of Use and that you understand and agree to abide by all of the provisions of the Client Portal Terms of Use, as they will be modified from time to time.  You will find the latest version of this agreement in the Terms and Conditions area of the portal. You understand the risks associated with using the client portal, including compromise of protected health information. You understand that the Client Portal is a free service and may be suspended or terminated at any time and for any reason.  If service is suspended or terminated, we will notify you as promptly as we reasonably can. You have had a chance to ask questions and have received answers to your satisfaction. If you don’t understand, or do not agree to comply with our policies and procedures, do not sign this agreement and do not request a username and password.
